Maintenance package Request Form
Each package is based on a monthly payment. Credit card will be stored on file and payments will automatically be done each month. 30 Day cancellation in writing prior to the end of the term. Term last for 12 months. If cancelled prior to end of term, the remaining balance will be billed in full. Scheduling to be done by customer. Any unused visitations do not carry over to the next term and will be forfeited. Package stays with the property, and is transferrable to new owner of property. Insurance options will be done by an Independent Agency licensed by the state, representing approximately 20 companies. Jeffers and Leek is not affiliated with the Independent Agency or other insurance companies. 20% off participating lighting galleries is good for light fixtures only at a lighting showroom. Showroom locations currently are located in Cranberry & Canonsburg.
